LAKELACE, England – Men in their 40s who’ve had erectile dysfunction have long been treated with a pill that promises to ease their symptoms. But now, a new study suggests men who take Viagra or sildenafil may have a higher risk of heart disease.

Men who’ve had heart attacks or strokes for at least a decade and are taking medication such as nitrates for chest pain or nitric oxide donors to help their heart remain healthy are at a particularly high risk of developing the conditions.

In the new study, men who took the drug sildenafil or vardenafil had an increased risk of developing high-density lipoprotein (HDL) cholesterol levels. High-density lipoprotein (HDL) cholesterol is a “good” lipid-lowering factor, but in men who take a lot of cholesterol-lowering medications, it can lead to plaque buildup in the arteries, which can increase the risk for cardiovascular disease.

“These findings are alarming and important to consider,” said Dr. David Brown, a cardiologist at University of California, San Diego. “These findings raise serious concerns about the potential for increased heart attacks and stroke risk, and there’s no way to know for sure whether these are related to erectile dysfunction.”

Viagra and sildenafil are both available in the U. S. and they are considered to be “little known” drugs. But their effects on the heart and the arteries are unclear, and doctors are not allowed to prescribe them.

“It’s important to know that this is not an isolated case, and that there’s not enough data to say whether it’s safe to prescribe sildenafil,” Brown said.

Viagra has been linked to an increased risk of heart attack and stroke. A study in 2016 estimated that men who take Viagra had an increased risk of heart attack and stroke.

In other words, Viagra and sildenafil may increase the risk of heart disease, but there is no way to know for sure whether the drug is safe for men who take it.

Brown and his colleagues found that in men who took Viagra, the highest incidence of heart attack or stroke was for those who took sildenafil. But the study was limited by a lack of control groups to measure for cardiovascular risk, as the data were not used to adjust for other possible confounding factors.

In the new study, they analyzed data from more than 600,000 men in the United States who took Viagra or sildenafil for at least five years and were taking sildenafil for the first time since their ED diagnosis. They included patients with ED symptoms such as high blood pressure, high cholesterol, or diabetes.

Of those who took the drug, about 90 percent reported a significant increase in heart attack risk with sildenafil. But that increased risk was not observed in the other drugs.

Brown said that while the study did not show a significant increase in heart attack risk for men who took Viagra or sildenafil, the risk was small. “In terms of the findings, we were just looking at what we could detect with a relatively small sample of men,” Brown said.

He added that he believed that the new data “was a good first step” in making an informed decision.

The study was funded by the National Heart, Lung, and Blood Institute and the National Institutes of Health and National Institute of Diabetes and Digestive and Kidney Diseases, and was published in the Journal of the American Medical Association.

“We are committed to improving the quality of life for these patients,” Brown said. “There are other treatments for ED and we will continue to work to make lifestyle changes more effective and prevent the progression of ED.”
